Submit A Claim

You must have these items ready before you submit your claim:

1. Bill of Lading (Or inland waybill, or any other proof of shipment)
2. Delivery receipt(s)
3. Invoices. Identify or mark damaged/short items
4. Photos of damage (s) (distant and closeup)
5. Other relevant information pertaining to the claim if applicable

Please note:

  • Damaged goods must be retained for presentation at time of settlement.
  • Depending on the type and extent of loss, other documents may be required.
  • If you had different shipments impacted by loss or damage, enter a different claim per bill of lading.

Any person who knowingly and with intent to injure, defraud, or deceive any insurer files a statement of claim or an application containing any false, incomplete, or misleading information is guilty of a felony of the third degree.
